2015 PuSh Festival Lifestyle Survey – Participate and Win!

Thank you for helping the PuSh Festival by taking this anonymous, confidential survey. It will take approximately 10 minutes to complete.

The survey pertains primarily to your lifestyle and habits. Your answers will be treated with the utmost privacy and confidentiality, and will not be attached to identifying personal information, nor used to market to you individually. Your answers will remain anonymous, and collected as a whole to provide an overall picture of the Festival audience. The general statistics we collect will be used to help us generate additional revenue from sponsors and supporters, and help the festival continue to grow.
